Application Forms:

Before you submit your application, the following is required to be included as part of your application:

1. A picture of the proposed improvement. This can be from a catalog or the Internet.
2. A site plan showing the entire property, the location of the home and location of the improvement. This can be a drawing from your contractor. Dimensions and distances from the side and rear property lines are required. You can find your lot on the Map section of the website or contact the Miami Township Zoning Department (Jeff Uckotter, Jeff.Uckotter@miamitwpoh.gov or Brian Elliff, Brian.Elliff@miamitwpoh.gov) for the side and rear zoning setbacks.

Also, please consult your documents for guidance.

Improvement Application

Instructions for submitting:

Please use the "Comments" page to submit your improvement application. You will need to register on the website first, and this will help us contact you in the future.

1) In the comments field please write "Improvement application".

2) Use the upload file feature to attach supporting documents.

We prefer this method for submitting applications because it ensures that everyone that needs to review the application receives it ASAP for review.
